Get used to this exceptions request form

The American Medical Association and America's Health Insurance Plans have developed a form for doctors to use to order nonformulary drugs under Medicare. This Exceptions Request Form is available at http://www.cms.hhs.gov/MLNProducts/Downloads/Form_Exceptions_final.pdf on the CMS Web site. They added that many health insurers are expected to use this form immediately.
