Bulk Rx ingredient supplier Spectrum Laboratory has recalled all lots of tacrolimus powder due to sub-potency in some lots. All pharmacies that have received Spectrum's tacrolimus powder should stop using it immediately and contact the company to make arrangements to return it. The catalogue number is T3192. The recall does not apply to tacrolimus in finished form as Prograf (Astellas Pharma) or to Prograf oral capsules that have been used for compounding.
