The American Society of Health-System Pharmacists (ASHP) Foundation is currently accepting applications for the 2011 Award for Excellence in Medication-Use Safety. Funded by the Cardinal Health Foundation, this $50,000 award recognizes the efforts of a pharmacist-led multidisciplinary team that has made significant improvements related to medication safety in its hospital or health system.
Initiatives will be evaluated against 5 criteria: medication-use system initiative scope, pharmacist leadership, planning and implementation, measurable outcomes and impact, and innovation and generalizability.
The recipient and finalists will be recognized at an awards ceremony at the opening general session of the 2011 ASHP Midyear Clinical Meeting, taking place in New Orleans December 4 to 8.
A representative of the recipient organization will take part in a media outreach during the 2012 National Patient Safety Week and will deliver an educational session about its initiative at the 2012 ASHP Summer Meeting, taking place in Baltimore June 10 to 13.
