NACDS Foundation, APhA to launch MTM Service Model Version 2.0 APhA, NACDS Foundation to offer new MTM model.
The American Pharmacists Association are planning to launch Medication Therapy Management in Pharmacy Practice: Core Elements of an MTM Service Model Version 2.0 on March 14. NACDS Foundation and APhA have been working together to develop a model framework for implementing effective MTM services in community pharmacy. MTM in Community Pharmacy Practice: Core Elements of an MTM Service Version 1.0 was launched n 2005. The 2008 update, Version 2.0, is designed to improve collaboration among pharmacists, physicians, and other healthcare professionals across healthcare settings; enhance communication between patients and their healthcare team; and optimize medication use for improved patient outcomes.
