All Pharmacies Are Not The Same

Tripp Logan, PharmD, and Jake Galdo, PharmD, discuss the nuances behind community pharmacy practices and the importance of understanding patient, policymaker, and provider perspectives.

In the evolving landscape of health care, 2 pharmacy leaders are challenging the long-held assumption that "all pharmacies are the same." Jake Galdo, PharmD, MBA, BCPS, BCGP, managing network facilitator of CPESN Community Health, and Tripp Logan, PharmD, vice president of SEMO Rx Pharmacies, discuss how patients, policymakers, and health care providers perceive pharmacy services. Galdo recalls struggling to find a COVID-19 vaccine for his 3-year-old daughter, ultimately discovering that not all pharmacies offer the same services or capabilities, which became the catalyst for a broader conversation about pharmacy differentiation.

Drawing parallels with other service industries like hotels, restaurants, and ride-sharing platforms, they argue that pharmacies should be evaluated on more than just product dispensing. They propose a comprehensive approach that measures quality, accessibility, and patient-centered care. With over 22,000 independent pharmacies and 21,000 chain pharmacies in the United States, the market is ripe for reimagining how these essential health care providers are understood and valued.

Their research highlights significant disparities in pharmacy reimbursement and service delivery. In one example from Georgia, an independent pharmacy was reimbursed $1.90 for a 90-day generic prescription, while nearby chain pharmacies received $46.87 for the same service. By championing a new framework that emphasizes quality over mere cost, Galdo and Logan are advocating for a patient-centric model that recognizes the nuanced role pharmacies play in community health. Their vision extends beyond traditional pharmaceutical dispensing, encompassing critical services like language support, chronic disease management, and addressing social determinants of health. As health care continues to evolve, their message is clear: not all pharmacies are created equal, and it's time to start recognizing and rewarding pharmacies that go above and beyond.
