Pharmacists Ensure Safety in Managing Patients’ Insulin Regimens | ASHP Midyear 2025

Diana Isaacs, PharmD, BCPS, BCACP, BC-ADM, CDCES, discussed potential safety concerns and the pharmacist’s role in managing insulin.

Despite the significant advancements made in insulin dosing and diabetes care as a whole, there is still potential for severe outcomes if regimens are not managed properly. From diabetic ketoacidosis (DKA) to other emergency-related complications, pharmacists are some of the most well-positioned providers for addressing these concerns.

“With insulin pumps, it is typically replacing both the background and the mealtime insulin,” Diana Isaacs, PharmD, BCPS, BCACP, BC-ADM, CDCES, director of education and training in diabetes technology at the Cleveland Clinic, told Drug Topics. “What that means is, if there's an interruption in the insulin pump for any reason—whether someone runs out of insulin, it's leaking insulin, or it stopped working—they could experience complications pretty quickly. It's important that each person has awareness of how to troubleshoot, that they have alarms set for high glucose levels, and they know how to take action.”

At the 2025 American Society of Health-System Pharmacists (ASHP) Midyear Clinical Meeting & Exhibition, Isaacs joined Drug Topics to provide further insights into the connections between pharmacists and patients with diabetes. While they have made their skills known in both the type 1 and type 2 diabetes spaces—among other cardiometabolic disease states—pharmacists have since exhibited their authoritative expertise in managing insulin regimens.

With increased knowledge on drug-drug interactions and the use of advanced medical equipment, pharmacists have been able to improve insulin-related outcomes while, more importantly, keeping patients safe and protected against emergency ordeals.
