
How Weight-Loss Drugs Could Reshape the Landscape of Addiction Treatment
With a slew of new GLP-1 indications approved in recent history, researchers and experts are now diving into their use in treating substance use disorders.
As new data and research on glucagon-like peptide-1 (GLP-1) medications are released, patients and providers alike are constantly uncovering new ways to utilize this drug class and improve outcomes on an immense scale. Now targeting the drugs’ ability to impact patients’ reward pathways, GLP-1s could be the future gold standard for treating substance use disorders (SUDs).
“GLP-1s act really as broad neuromodulators,” Sylvie Stacy, MD, MPH, chief medical officer at Rehab.com, told Drug Topics. “They act on the brain's reward sensitivity, and because of that, they show potential across a wide range of substances.”
On this week’s episode of the Over the Counter podcast, Stacy explains that these medications impact the brain significantly, potentially more so than substance-specific treatments. By targeting the brain's mesocorticolimbic system, GLP-1s dampen the supra-physiological dopamine spikes that drive compulsive use, effectively muffling “drug noise” without harming the brain's ability to experience natural joy.
Current data is strongest for alcohol use disorder, showing reduced hospitalizations and cravings, while nicotine trials suggest GLP-1s remove the barrier of cessation-related weight gain. Early evidence even indicates a lower risk of opioid overdoses.
Lacking FDA approval for addiction, however, providers often utilize “dual-benefit prescribing,” treating metabolic issues while monitoring comorbid cravings. Pharmacists are essential for distinguishing gastrointestinal side effects from withdrawal symptoms and managing slow titration to ensure long-term stability.
Stacy’s verdict remains optimistic but cautious. The field requires phase 3 trials to determine specific addiction dosing, which may differ from obesity regimens. Furthermore, until official indications are secured, insurance coverage and high costs remain significant hurdles to standardizing GLP-1s in addiction care.
