The FDA approved AstraZeneca’s baxdrostat (Baxfendy) as the first and only aldosterone synthase inhibitor for the treatment of hypertension in adults. This first-in-class oral medication is indicated for use in combination with other antihypertensive treatments to lower blood pressure in patients whose condition is not adequately controlled by existing therapies.1

“Hypertension remains a staggeringly widespread silent killer and a leading risk factor for stroke, heart attack, kidney damage, and dementia,” John Clymer, executive director of the National Forum for Heart Disease and Stroke Prevention, said in a news release.1 Tens of millions of people struggle to control their blood pressure despite lifestyle changes and currently available treatments. For pharmacists managing complex medication regimens, the approval represents a novel pharmacological approach to a disease that has seen little therapeutic progress over the last two decades.

Clinical Data for Baxdrostat

Baxdrostat targets the production of aldosterone, a hormone produced in the adrenal gland that raises blood pressure to unhealthy levels by promoting sodium and water retention. Although traditional treatments often focus on blocking receptors, this small molecule specifically inhibits aldosterone synthase, an enzyme encoded by the CYP11B2 gene, thereby addressing a root cause of persistently elevated blood pressure.

Clinical trials observed that baxdrostat significantly lowered aldosterone levels without impacting cortisol levels, demonstrating a high degree of selectivity that is vital for patient safety.

The clinical evidence supporting this approval stems primarily from the BaxHTN phase 3 trial, a multicenter, randomized, double-blind study involving 796 participants. The trial focused on individuals with uncontrolled hypertension—those on a stable regimen of 2 medications, including a diuretic—and those with resistant hypertension, defined as being on 3 or more medications, including a diuretic. Results published in the New England Journal of Medicine showed that a 2 mg dose of baxdrostat achieved an absolute reduction in mean seated systolic blood pressure of 15.7 mmHg, which represented a 9.8 mmHg reduction when adjusted for the placebo.1,2

At the 12-week mark, the 1 mg dose also showed meaningful results, with an absolute reduction of 14.5 mmHg and a placebo-adjusted reduction of 8.7 mmHg (p<0.001). For comparison, the group receiving the placebo saw an absolute reduction of 5.8 mmHg.3

Baxdrostat was generally well-tolerated, according to the study authors. The drug significantly lowered aldosterone without affecting cortisol levels across a range of doses. Serious adverse events occurred in 1.9% of patients taking the 1 mg dose, 3.4% taking the 2 mg dose, and 2.7% taking the placebo. Most adverse events were mild and included hyperkalemia, hyponatremia, hypotension, muscle spasms, and dizziness.

What Pharmacists Should Know

From a clinical practice perspective, these reductions are significant because even a 10 mmHg decrease in systolic blood pressure is associated with a roughly 20% lower risk of serious cardiovascular events. In the United States alone, approximately 23 million patients suffer from hypertension that remains uncontrolled despite being on 2 or more medications. This persistent elevation is a leading risk factor for life-threatening complications, including heart attack, stroke, chronic kidney disease, and vascular dementia.1,4

Pharmacists should note that baxdrostat was generally well-tolerated in clinical settings, with no unanticipated safety findings reported during the trial phase. Beyond its primary indication, the drug is currently being investigated for other conditions where high aldosterone levels drive cardiorenal risk, such as primary aldosteronism and in combination with other agents for chronic kidney disease.1

As health care providers look toward better ways to protect vital organs and slow disease progression, the introduction of this aldosterone synthase inhibitor provides a much-needed tool for patients who have not responded to or tolerated standard antihypertensive treatments.
