
Medication Management Lags Due to Fragmented Data Systems

Despite digital transformation modernizing hospital administrative systems like billing and electronic health records (EHRs), medication management remains stuck in the past. EHRs are primarily documentation tools that record order entries and confirm whether a patient received a drug, but they fail to track the physical journey of medications entirely.
“The EHR stops at what medication is [a patient] going to get, and did [they] receive it or not; that’s it,” Rick Couldry, MS/HSPA, senior vice president and chief pharmacy and clinical officer at Omnicell, told Drug Topics®. “How that medication got into the health system, where it was stored, how long it was stored there, where it was distributed to, who moved it from place to place, how many do we have in every location; I could go on and on. The physical following of all those medications in a health system ecosystem is much more complicated than what the EHR is built to capture.”
This tracking gap becomes increasingly dangerous as modern health systems expand. Medications are no longer confined within hospital walls but are widely distributed across clinics, procedural suites, radiology units, ambulatory programs, and patients’ homes. Traditional, location-bound transaction software was built simply for counting pills and managing local inventory, failing to support clinical patient relationships.1
Lack of integration creates severe accessibility barriers for patients in rural or underserved areas, leading to gaps in care and medication non-adherence. Despite telehealth adoption in the US surging from 11% in 2019 to 46%, medication management still struggles to match this remote digital agility. Consequently, remote clinical pharmacists struggle to coordinate care across these fragmented physical and digital networks.1,2
