Every Visit Is a Chance to Care: How Pharmacy Teams Can Catch More Care Opportunities at the Counter

Sponsored content from PQS by Innovaccer

EQUIPP Copilot™ puts care opportunities in front of your team while the patient is still at the counter, in the pharmacy management system you already use.

Caring for our communities is what community pharmacies do every day. Patients come in for a refill and leave with a question answered, a concern addressed, or a reminder that someone is paying attention to their health. For many patients, the pharmacy counter is the most familiar place in their healthcare.

This American Pharmacists Month, PQS by Innovaccer recognizes the pharmacists and pharmacy teams behind that care. It is also a good moment to ask a practical question: how many care opportunities walk up to the counter each day, and how many does the team get to act on?

Why Opportunities Get Missed

Most care opportunities surface during the busiest part of the day. A pharmacist handling a pickup is verifying prescriptions, counseling, and processing the transaction at the same time. Checking whether that patient is due for an adherence check-in or a blood pressure reading usually means stepping away to look it up in another system.

That extra step is where opportunities slip through – the intent to help is there, but the information is not readily available to the team.

Care Opportunities in the Workflow

EQUIPP Copilot™ is built to address that gap between knowledge and action. It runs alongside the pharmacy management system your team already uses, with no IT integration and no change to how prescriptions are filled. When a team member opens a patient profile, Copilot surfaces care-gap prompts, adherence alerts, and program eligibility for that patient, so the conversation can happen while the patient is still at the counter.

Program documentation is available within the workflow, and a population-level view helps teams plan outreach and monitor overall performance between visits.

Care That Payer Programs Support

Many of these opportunities connect to services pharmacies already deliver, and payer programs available through the EQUIPP® platform offer reimbursement for that work. Current program types include:

  • Adherence monitoring, with quarterly patient check-ins, Extended Days’ Supply conversions, Final Fill, and statin gap closure
  • Blood pressure gap closure, with readings documented as a point-of-care service
  • A1C gap closure, for pharmacies with a CLIA waiver

Blood pressure and A1C screenings can be performed by any credentialed staff member, so the work can be shared across the team rather than resting on the pharmacist alone. Program opportunities vary by payer and plan year, and Copilot shows which ones apply to the patient in front of you.

Results From the Field

EQUIPP Copilot™ pharmacies closed 1.5 to 2 times more adherence gaps earning 4x more clinical care revenue than other program users. Same programs, same patients, same clinical work. The difference was having the right information in front of the team at the right moment.

Every visit is a chance to care. Thank you to every pharmacist and pharmacy team member that takes the time to care, and we wish you a happy American Pharmacists Month.
