APhA Foundation to work on chronic care with employer group

The APhA Foundation has partnered with the National Business Coalition on Health (NBCH) to help patients better manage chronic health conditions. NBCH is an organization of more than 70 employer-based healthcare coalitions, representing more than 10,000 employers across the nation. NBCH will implement APhA Foundation's HealthMapRx, an R.Ph.-coordinated, patient self-management program. For employees with diabetes, employers provide incentives to participate in the program, including waived Rx co-pays and free diabetes supplies. NBCH will develop resources to assist local business coalitions in implementing the HealthMapRx program.
